  • Upwards of one incidence of Unsafe Driving BASIC, as defined by the Federal Motor Carrier Safety Administration’s Compliance, Safety, and Accountability (CSA) program,7 in any of the following Groups: Only work or construction zones, Bad Driving, or Exceeding the speed of limit.   1. During the 36 months preceding the application date, there was no citation, record, or conviction of:
  • Having upwards of 2 moving offenses is a serious offense.
  • In any combo, more than one avoidable accident, or even more than one avoidable accident and one moving infraction
  • Driving an automobile without a proper permit or an incorrect license, including driving while a license is suspended, revoked, or canceled.
  • For felonies resulting in the revocation of more than one driving license for driving violations or moving infractions, the penalty is 30 days or more.
  • Endangering any vehicle in a careless, forceful, or negligent manner
  • Any occurrence of surpassing the maximum speed limit by 15 mph or more while driving a commercial motor vehicle (CMV)7 without being clinically approved
  • Contravening the law by driving a car that includes alcoholic beverages, narcotics, or restricted substances
  •   Any offense results in the suspension of a driver’s license owing to excessive alcohol consumption.   1. During the 60 months preceding the application date, there should be no citation, record, citation, or sentence of:
  •  Driving a car and being intoxicated, under narcotics, or illicit substances are illegal.
  •  Refusal to comply with or satisfy any narcotic, banned substance, or liquor testing required by the United States Department of Transportation (DOT) or demanded by any police officer concerning driving any automobile.
